After you are certain you meet the eligibility requirements for the program, you may apply for the BASE-TN program by mailing a complete application packet by the established deadline to:

The application packet must include the following components.
- Letter of Intent (Cover Letter-see item 30 on application form for additional information)
- BASE-TN Application (available on this website)
- Official College Transcript(s)
- Four Letters of Recommendation (see Item 29 on application form for additional information)
- Letter of Acceptance to Graduate Studies/Undergraduate Studies
- Standardized Test Score Reports (ACT of 22 or higher; GRE required for graduate studies)
- Copy of Tennessee Teaching License (if currently licensed)
** A plan of study will be developed for all applicants who are admitted to the program. **
