  • Social Security Number
  • Dates in Attendance at UTM
  • Address to Mail Transcript
  • Date of Birth
  • Signature

Fax Requests:

  • Fax requests, with the above information, to: (731) 881-7048
  • Only unofficial copies of transcripts will be faxed

 

Cost:
  • No charge for mailed transcripts, up to 10 copies/calendar year
  • Additional copies $2.00/copy
  • $5.00 per (unofficial only) faxed copy 
    (Cash, MasterCard, Visa, or Discover accepted)
