Faculty Leave Proposal Evaluation Forms

This proposal is being evaluated on the following 5 point, Likert-type scale where 1 = "strongly disagrees" and 5 = "strongly agrees." Total points will receive an overall rating from 0-4. Individual proposal ratings are confidential. The rating sheets will be destroyed June 1.

1. A fair proposal: Parts of this proposal are good, but it is one for which you have substantial reservations.
0. A poor proposal: This proposal does not warrant consideration.
