Class Teaching Schedule

To schedule the in-class evaluation and determine the number of evaluation forms that will be needed, we request that you provide to us the following information. This information and packets of evaluation materials will be sent to the chair of your academic until so that he/she can schedule the evaluations. Please note that only 2 of your classes/labs will be evaluated and this will not be on test days. Therefore, you must provide a copy of the syllabus for all classes you teach.
