Health Connect America offers In-Home Family Preservation services that are short-term, highly intensive, home-based interventions designed to protect, treat and support families with at least one child at imminent risk of being placed in custody. These services may also be used to reunify a family when a child has been in an out-of-home placement. This level of intervention is needed to enable the child to return home and successfully remain there. Health Connect America offers a wide, comprehensive range of flexible and responsive interventions tailored to the individual child's and family's needs. SHIP stands for State Health Insurance assistance Program. We provide free and objective one on one counseling, information distribution and assistance to people with Medicare, potential Medicare consumers, their families and other advocates. SHIP began in 1992 and since has grown from a one topic consumer counseling program to one that covers a multitude of topics and issues. Topics typically addressed are on Medicare and Medicaid, Medigap, enrollment in Medicare prescription drug plans and Low-Income Subsidy, finding help with the cost of prescription drugs during coverage gaps and access to drugs not covered under Part D. SHIP may not endorse any insurance company, agency, agent or plan—to the extent that we are prohibited from making even the appearance of an endorsement.
Our volunteers can be used to answer telephones, data entry, distribute materials at health fairs, distribute pamphlets at commodities day in various counties and counseling beneficiaries on Medicare topics. It would be their choice as to what they would want to do as it pertains to what we may need at the moment. October 15 through December 7th is Medicare Part D open enrollment and we need more counselors during that time than any other time of the year
