FIELD ( ADDRESS )

Dear (Teachers name ) :

The Mathematics and Statistics Department of The University of Tennessee at Martin will sponsor the annual Junior High School / Middle School Mathematics Contest on Thursday, April 29, 2010.  The contest will include sixth, seventh and eighth grade divisions.  This mailing refers to all three divisions.

Contest information, an enrollment form, tentative contest schedule, campus parking map, and Boling University Center map are included in this mailing.  If your school intends to participate in the contest, your enrollment form should be mailed no later than Friday, April 09, 2010; to the address provided on the back of the enrollment form.  Because of printing and mailing costs, you will not be notified by mail of our receipt of your enrollment form.  However, if you will include your email address on the form, I will notify you by email of receipt of your form.  If you prefer you can check the contest web page; I will keep a list of schools currently registered, the contest schedule, and other details on the contest web page.  The contest web page is http://www.utm.edu/~desireem/contests.html.

Each student is assigned a five digit number that should be used as their identification on their exam.  Your school code number is :        .  Your student’s identification number is found in the following way:  The first digit refers to grade level (6, 7, or 8); the next two digits are your school code; and the last two digits are the student number on the enrollment form for their grade (01, 02, 03, …).  For example, if I am taking the seventh grade test, my school site number is 78, and my name is on the line numbered 6 then my identification number for the contest is :  77806 .

The contest will be held on The University of Tennessee at Martin campus in the Boling University Center.  A registration table will be located in the foyer of the Watkins Auditorium which is in the northwest corner of the University Center on the first floor.  Please plan to stay for the presentation of awards as all participants will receive mementos for participating in the contest.

The test fee is $5.00 per contestant.  We prefer that you send payment (in the form of a check) with your enrollment form.  Please make the check payable to “UTM Math and Stat Department”.  After testing begins, additional tests will be sold at $1.00 per copy.

Calculators will be allowed.  Students may use any calculator that does not have a QWERTY keyboard.  There will be 30 questions, and the scoring formula will be 4R – W + 30.  Testing time will be 60 minutes.

Registration will be held from 8:00 to 9:15.  We encourage you to arrive for registration as early as possible.  We look forward to seeing you on April 29, 2010.  If you have any questions regarding the contest then please do not hesitate to contact me.
