Developmental Factors in Infancy and Childhood
You are being asked to answer questions regarding your child/children's development. Please fill out 1 form for each of your biological children. The questionnaire is very short and should only require a few minutes for each child.
The information you provide concerning your child or children is for research purposes only. No identifying information will be used and your participation is strictly voluntary. Please select a fictional birthdate for your confidential ID#. This does not have to be one of your children's birthdays. Use the following format: 01/09/02. This ID# should be used for each questionnaire you complete. Remember, complete 1 questionnaire per child.
