Fill out Form DSP-11 (tan
application) if:
- you’ve never had a passport before
- you lost you previous passport
- you do not meet all requirements
necessary to use the pink (DSP-82) application.
In order to apply, you need:
- a completed passport application
- 2 official passport photos taken within
the last 6 months
- proof of U.S. citizenship (original or
certified copy of your birth certificate, previous U.S. passport or
naturalization papers.)
- a picture ID (valid U.S. driver’s
license, state ID, etc.)
- a check or money order made out for the
correct fee (payable to Passport Services), the cost is $85.
