Request a Replacement Skyhawk Card

If your Skyhawk card is lost or stolen, you may request a new card for a small fee of $25. To request a replacement card you may either submit a request online or come to any cashier window in the Bursar's Office.

Online

  1. Log on to myUTMartin portal
  2. Click on the Banner tab at the top of the screen.
  3. Click on the PERSONAL tab at the top of the screen
  4. Click on SKYHAWK LOST ID CARD REPLACEMENT
  5. Read the terms of replacement
  6. Click on REQUEST REPLACEMENT SKYHAWK ID CARD (25 DOLLAR CHARGE)
  7. Your request will be submitted. You may either pay for it online using the PAYMENTS AND DEPOSITS link or pay in person at the cashier window in the Bursar's Office

In Person

You may come to the cashier window in the Bursar's Office, where you can make a request and pay for your replacement card.

1098T

1. Log into your MyUTMartin portal
2. Under UTM Apps, click Touchnet
3. Look under Statements on the homepage
4. Click View Statements next to "Your latest 1098-T Tax statement"
5. Click View

*If you click "View" and your statement does not open, please go to the upper right corner of your browser and ensure that your pop-up blocker is disabled.*
